3. COMMIT to raising minimum sponsorship for Herzog - Israel Center for the Treatment of Psychotrauma.
4. When you receive a link to your personl page, JOIN our website and fill out your information and write a few words about yourself.
5. SUBMIT your completed medical form and a copy of paid medical / travel insurance. Scan and email, fax, or post it to reach our office in Jerusalem before March 1, 2010. Please keep the original.
